2. Find a Doctor or Facility with 3 different search options
   1. Doctor by Type: Select or enter a specialty or type of doctor
   2. Doctor by Name: Search providers by first or last name
   3. Locations: Search hospitals, facilities, labs or pharmacies by type or name
      For all of the above, click the magnifying glass to continue
3. Please Select a Plan: enter the location in which you live, and press continue
4. Finally, “Select a Plan”, click PPO, Choice Fund PPO and search results will appear
   
   At any point, if it prompts you to log in or continue as a guest, please continue as a guest
